I am wanting to add a confirmation page basically asking people to confirm certain information before they are able to move on and open the adoption application - is this possible?

      Here are some ways of doing this.

      1) Create a Separate Web Page with information
      Some organizations create a separate web page with information an applicant should read before they can access the adoption application.

      2) Create individual FAQ's as Questions on Form
      Another option is to create a series of questions on the form itself, with a person adding initials, or a required answer with a radio or drop down to select "Yes" or "I understand". 

      3) Use the Form Introduction
      Another way organizations do this is to add content to the "Form Introduction", which will appear before they fill out the form. 

      4) Create a Question with the Answer Type "Content" followed by a confirmation question "Yes"
      Some organizations add one question, using the "Answer Type" "Content" which allows them to add a paragraphs of text to the form itself, such as several FAQ's.

      Then immediately following this Content, add another question asking the applicant to select "Yes", or "I understand" required. 

      Example of number 4:

      • This is an example of a Foster Application with FAQ's that an organization would like applicants to read before applying. The FAQ's are added directly to the form, followed by a drop down for applicant to select they have read these FAQ's. This helps ensure that the applicant has read important information before applying. 

      • A form question was created with the Answer Type "Content".

      • The Answer Type "Content"allows your organization more control over how content some types of content is displayed on your form. The screen capture below shows how Heading 1 is used for the text "Frequently Asked Foster Questions", and a horizontal line is used to separate each FAQ to improve readability. Content is useful if you want to separate content with Headings, such as Heading 1Heading 2 for subheadings, and Normal for paragraph text. 

      Next a question is added asking if the applicant has read the Foster FAQ's "I have read the Foster Program Frequently Asked Questions." The Answer Type is "Drop Down Select", and the Select Group is "Yes or No". This is set to Required. A radio button could have also been used.
