How can I ask for an applicant's age on my adoption or foster application?

I understand that certain sensitive personally identifiable information cannot be asked online. How can I ask for an applicant's age or birthdate on our online form?

    Here is the link to our online application where we ask quite simple for the ages of all members of the household:

  2. Barbara Burchard

    The rescue I work for asked for DOB first and switched to age - Rescue Groups no longer allows either as they feel it violates their policy.   We have now switched to age ranges.

    You may ask for an age range.

    You can create a question such as:

    "What is your age?"

    And then create a select group with the following options:

    • 18-24 years old
    • 25-34 years old
    • 35-44 years old
    • 45-54 years old
    • 55-64 years old
    • 65-74 years old
    • 75 years or older
