What is a real world example of using forms for something other than adoption and foster applications?

      Many organizations create Adoption Applications, Volunteer Applications, and Foster Applications.

      Applications are meant to be a tool that allows you to review an applicant and their questions prior to approval. While some organizations also use forms for post adoption follow ups, you must fill out the form on the back end and not the front end, so that the form is connected to the adopter and not the volunteer.         We also use them for volunteer applications, owner surrender initial application and Vet / Home Visit recheck forms for approved adopters who have moved, added pets, or need updated because it has been two years since approval.
