How do I get a URL so my webmistress can put an online form on the Volunteer page of our website (website not through Rescue Groups)?

LAWS set up its website before we knew about Rescue Groups.  We are using 3 online forms from our RG account on our website:  adoption, foster and volunteer application forms.  I wish to add a Junior Volunteer application and need the url to give to our webmistress to put in on our site for poeople to use.  A previous board member worked with those 3 forms, but I don't know how to get the correct URL to make the form exportable .  Choosing the "Export" option (any of the 3) does not work.
