Why are Name and Email not displayed on PDF version of Online Form?

I need to create a PDF version of my adoption application so that I can print it, but the PDF version does not ask for the applicant's name or email address. Is there a way to fix this?

      Michele, can you check the Contact fields that are selected for the Online Form?  Those fields should be requested on the Online Form.

      Update: We've confirmed that this is an issue with the PDF Online Form.  We're working on getting it fixed, and the Issue is linked below:

        Name and email are requested on the online form, it's the PDF version that is missing it.  The contact fields for Name and Email are not editable by me, website says "Name and email address are always collected and required," so I don't think anything I did could have deselected them.

        This issue should be resolved now. Both Name and Email address should always shown on the Online Form PDF. If you notice any problems, please let us know.
