Follow these instructions to create an online form for someone who is already in your Contacts list.

  1. Log in to your account at RescueGroups.org
  2. Go to Contacts > Submitted Forms
  3. Click Submit a Form button at the top of the page
  4. Select a Contact from the Contacts dropdown, or click the Add a Contact button to the right of the dropdown to create a new contact
  5. Select an Online Form
  6. Click Submit Form
