You can make a form public by editing the form and next to the question "Is this a public form?" select "Yes."

I went to the create form page to see how it is done, but when I click on the create form the page that pops up is different from the one shown. It does not list is this public form, Allow multiple submissions, unregistered users etc. the one that is coming up for me is short.
