How can an applicant upload a photo to an owner surrender form?

I am creating an owner surrender form and would like to ask the owner to upload a photo of the animal he/she wants to surrender.  Can this be done?  If yes, how?

      Is this feature available yet?  What is a work around to allow people who need to surrender their pets to send pictures and videos.  This is a very helpful feature to make things easy on us and the consumer.  The below website has that feature and it is understandable helpful.
