How do I use the outcome feature to do a second intake? Keep getting the message record already exists?

I read that there needs to be an outcome - which does not show. None of my entries show outcomes.
In the past I have had the same problem - and get the same message. I was only able to process a 2nd intake successfully two times - and followed the same steps

So… first question - how do I add an outcome? Second question… is there an easy way to go back and add outcomes to all the dogs without outcomes?

The dog I am concerned about now is XXXX. Here’s what I have done:
Changed adoption status from success to return
Changed status to Available, then changed to Pending (because he is in a foster to adopt home)
Added surrenderer to owner’s group list
Tried to do an intake with the previous owner as the
surrendered

Last step is where I had the problem -

      Here's how to add an outcome to XXXX: 

      • Find XXXX, and click on his name.
      • At the top of that animal record page, click Outcomes
      • Click Add an Outcome for this animal.

      For the rest of the animals, it might be easier to go to Animals > Outcomes
      You can click on Open Intakes and process Outcomes there since all of the animals that need outcomes are listed there.


      Moving forward, every time you have an animal adopted, here's the best practice for how to do this:

      1. Enter the Outcome: Adopted
      2. Then process the adoption like you normally would (About animal adoptions).  

      The Intakes/Outcomes feature is designed based on the Asilomar reporting. Some organizations use that, while others do not. So if that's not something your organization needs, then you could skip the intakes/outcomes -- but it can be very helpful.
