Link My Forms to submitted adoption applications assigned to me

I have a number of submitted applications assigned to me, but nothing shows under My Forms.   How do I link them?

    Michelle,

    I have no problem with the link for one of the rescues I volunteer with.  It is the new rescue where I just show the number zero under My Forms.  I wanted to find out what needed to be done on the new rescue site.  

    Works on Eleventh Hour Rescue, 956
    Does NOT work on NorthStar Rescue, 8444
