How can I add a long document of questions to the end of an Online Form?

I am trying to add our foster and volunteer agreement to the end of the online form. I want to have boxes that need have the user initial and I don't know how to set that up.

      There are some options:

      A) You could leave the agreement out of the application. Once the applicant is approved, you could then use the pdf contract feature to pre-populate with contact information (from the form) and then email that pdf to the volunteer to esign or print and sign and send back to you.       B) You could include some of the text from the agreement to the form as "content".  Then add in questions that need to be initialed as "free form text short".

      C) You could include all of the text as "content". Then create one question where a user can choose a select group with only one answer "I agree".

      You could also use a combination of the above.
