How to add a logo to the top of our forms?

How can I add our organization's logo to the top of our form?  I've tried doing so via a couple of CSS codes but doesn't seem to take. Concern has been expressed over our organization's presence not being clear on the form (e.g. people being uncomfortable submitting info into a third party website type thing) and would like to make it clearer.

      You can add content to the Form Introduction by going to 

      • Features > Online Forms
      • Click on the "Name of the Form"
      • Next to Form Introduction, click "View and update the form introduction"
