Where should I add medical info? (Intake Service versus Journal Entry)

It looks like medical info can be entered as an Intake Service (Exam, Spay/Neuter, etc) or as a Journal Entry (Journal Entry Types: Spay/Neuter, Vaccination, etc).

What is the best practice for maintaining the medical info?  Only use Journal Entries?
