adoption page request - add fields

The first bullet is a repeat request (it must mean I REALLY WANT IT!):

• Add “Adoption Location” with custom locations so we can differentiate location adopted from (WHICH Petsmart or Petco or other store) from the lead
• Add “Adoption Counselor” to adoption screen, with link to contact list
• Add “Follow Up” fields, with comments section (1^st week follow up, 2^nd and 3^rd if possible)
