Sharyn, it doesn't look like the field from the Submitted Form is available in the Contacts > Submitted Forms table. I have added the "requests" label to your question so we can look into adding it. In the meantime if you click the Form name (like "Adoption Form") under the Form Name column it will take you to the Submitted Forms, and the Date is shown there.

Is there a way to see the date a form was submitted while in the contact file. You can see the form is there and link to the form to open it, but I don't see where the date the form was submitted shows up. Example, I have the name of a person, but don't know when they submitted the form. I search contacts and find two of the same last name. Each has submitted a form, but when? I need the most recent entry. I didn't see a submission date under "Add Data Table View".
