Populating Subject Line with Submitter's Name for Online Forms

We have several online forms. One is an Intake-Surrender Form. Is it possible to extract the name of the dog or the name of the person submitting the form and populate the subject line?

For applications, could we extract the submitter's name?

    OTOH, when all the form submissions have exactly the same subject, they're getting tracked as a single conversation, which is confusing!  Filters can match parts of subject lines...
