  1. Log in to your account.
  2. Navigate to Online Forms.
  3. Select Add an Online Form. You can follow along with this under the "Adding a New Form" in our guide here: Adding, editing, and deleting Online Forms 
  4. Create the structure of the form by selecting from the options presented, including filling out the email you want to send to those who submit this form.
  5. Add questions to your form (also adds them to the database to use later).  Find more details in this guide: Adding online form questions to your account
  6. Enable the form. (You can disable it again at any time.)).  More details here: Enabling or disabling a form 
  7. Edit the form and form questions if needed and save it.  More details found here: Editing and deleting online forms questions from your database
